Thank you for choosing Aasar Family Dentistry. We welcome Indiana Medicaid patients. To ensure a smooth scheduling process, please review the following information and verify your dental coverage before booking your appointment.

Accepted Indiana Medicaid Dental Plans

• DentaQuest/Anthem
• CareSource Indiana
• MDwise / Delta Dental
• Envolve / Centene Indiana

If your plan is not listed above, we are unfortunately not in-network for your Medicaid coverage.

Verify Your Dental Coverage

Many patients mistakenly provide medical insurance cards. Please confirm you have an active dental plan under one of the accepted providers:

1. Locate your dental insurance card.
2. Call Indiana Medicaid and ask: “Do I have active dental coverage through DentaQuest/Anthem, CareSource, MDwise/Delta Dental, or Envolve/Centene?”
3. If coverage is inactive, activate it before scheduling.

What to Send Us

• A photo of your dental insurance card (front and back)
• Your full name and date of birth
• Your Medicaid ID number
• The exact name of your dental plan

We will verify eligibility prior to your visit. Ineligible or inactive plans may result in appointment cancellation.

Self-Pay & Discount Plans

If you do not have Medicaid dental coverage, we offer affordable self-pay rates and a membership discount plan. Ask our staff for details.
